6. Health Information
We do not provide health services but only provide a therapeutic living environment, along with alcohol risk assessment and education for those charged with DUI/DWI and recovery coaching. When you provide information via our Contact Us form, do not include any Protected Health Information (“PHI”), such as your past, present, or future physical or mental health or condition and any related health care services.
If we require PHI to provide services to you, we will share with you a copy of our PHI privacy policy as required under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) prior to requesting your PHI. We may need PHI to coordinate with your physician, counselor, hospital, or others outside our program. In the event your insurance or other payor might pay for our services to you, we also may need your PHI to receive such payment.
7. User Rights
We process your personally identifiable information only as necessary to offer and provide services to you. Our legal basis for processing includes:
